Form 2 is to be filled out by the master of a Bermuda registered ship in order to inform this Administration that some details of the CSR require to be amended.  Once the form is completed it can be submitted electronically to   This e-mail address is being protected from spambots. You need JavaScript enabled to view it  and a paper copy placed in the CSR record file on board.  This becomes a valid legal record for 3 months which gives this administration time to complete the new document and forward it to the ship.

This is the new version of the Application for a Safe Manning Document in .pdf form.   This version requires the applicant to propose a manning level with justifications where necessary and also allows for the issue of multiple Safe Manning Diocuments when a vessel is likely to alternate between different modes and this is appropriate.
