ARF1 should be filled out by the Master or Safety Officer and submitted by e-mail to this office. This form should be submitted as soon as possible but in the case of a major accident resulting in injury it must be submitted not more than 24 hours after the accident.  The form may also be submitted as a record of a Dangerous Occurrence or "Near Miss" at the discretion of the Master or Safety Officer.
